Community-based Rehabilitation for those who have had a brain injury is a time-limited intensive package of rehabilitation

A team of specialist brain injury therapists and support workers will work with the client to achieve specific rehabilitation goals targeting the individuals specific needs.

Rehabilitation is provided directly to the person in their home and utilises community services. This is often more cost-effective than an inpatient service and allows clients to practice and see the relevance of compensatory strategies applied directly within their home.

Community Based Rehabilitation provides a team of specialist brain injury therapists and support workers, led by a case manager to:

  • Conduct a multidisciplinary assessment to identify the clients needs.
  • Work with the client and their family to set goals, which will improve community reintegration.
  • Deliver packages of rehabilitation to achieve these goals. These may include: memory training, anger management, problem solving and social skills.
  • Increase family understanding of client's needs and provide training for family and friends to deal with behaviour and memory problems.
  • Access community facilities (e.g. college courses, clubs, local gym, voluntary work, employment retraining) on which the client may rely long-term, and which can be introduced during rehabilitation.
  • Practice travel and social skills within the community.

Unlike case management service, Community Based Rehabilitation is not a long-term package. The overall aim is to achieve community reintegration and decrease the need for future services by increasing level of function and facilitating individual, vocational and family adaptation.


How do we provide community based rehabilitation?

Following a referral for community based rehabilitation we find a case manager who we believe is equipped to do the job in terms of qualifications, experience and personality. The case manager will conduct an assessment of the client's needs by reviewing the relevant records and meeting with the client and a close family member.

From this we identify the client's key requirements and determine whether further assessment from the multidisciplinary team is required. For example speech and language therapy / physiotherapy.

We try to ensure that the case manager and rehabilitation team member are local to the client, so that they have local knowledge and be on the spot when needed. For areas where we are able to offer this service, please see map. Once the client has been fully assessed we are able to prepare a proposal. This details our recommendations as regards the package of community based rehabilitation.

In order to make a referral for case management we require a written referral accompanied by any relevant medical records and case reports. On receipt of this information, which we shall acknowledge and start to progress the case.
